چکیده

Traumatic brain injury is a substantial cause ofmorbidity and mortality in the UK. An estimated 11 000 people per year sustain a severe traumatic brain injury, mostly between ages 15 and 29 years. Patients with severe traumatic brain injury have a high mortality rate (30-50%) and many survivors will have persistent severe neurological disability. Prompt identification and appropriate early management of traumatic brain injury is essential to optimise outcome, however, few guidelines are available for clinicians on management in the challenging prehospital environment. The 2007 report by the national enquiry into patient outcome and death on the provision of trauma services in the UK highlighted concerns about prehospital management of airways and ventilation in 15% of patients with severe head injuries. Doctors from various hospital specialties or general practice may be involved in providing prehospital care, so this review aims to provide a generic overview of available evidence on management of severe traumatic brain injury in adults in this environment.

Direct transport within an organized state trauma system reduces mortality in patients with severe traumatic brain injury.

BACKGROUND Prehospital management of traumatic brain injury (TBI) and trauma system development and organization are aspects of TBI care that have the potential to significantly impact patient outcome. This multi-center study was conducted to explore the effect of prehospital management decisions on early mortality after severe TBI. METHODS This report is based on 1449 patients with severe TB...

Outcomes following prehospital airway management in severe traumatic brain injury.

BACKGROUND Prevention of hypoxia and thus secondary brain injury in traumatic brain injury (TBI) is critical. However there is controversy regarding the role of endotracheal intubation in the prehospital management of TBI. OBJECTIVE To describe the outcome of TBI with various airway management methods employed in the prehospital setting in the Cape Town Metropole. METHODS The study was a co...

Experience in Prehospital Endotracheal Intubation Significantly Influences Mortality of Patients with Severe Traumatic Brain Injury: A Systematic Review and Meta-Analysis

BACKGROUND Patients with severe traumatic brain injury (TBI) are at high risk for airway obstruction and hypoxia at the accident scene, and routine prehospital endotracheal intubation has been widely advocated. However, the effects on outcome are unclear. We therefore aim to determine effects of prehospital intubation on mortality and hypothesize that such effects may depend on the emergency me...

Prehospital Interventions in Children with a Severe Traumatic Brain Injury

The impact of prehospital care on the outcome of patients with a traumatic brain injury has investigated in Sweden and the results from Rudehill et al. [14] showed a positive impact on postresuscitation neurological status and therefore better neurological outcome because of an effective prehospital care [13]. Prehospital care has increased in terms of both medical and technical competence [14]...

The study protocol for the Head Injury Retrieval Trial (HIRT): a single centre randomised controlled trial of physician prehospital management of severe blunt head injury compared with management by paramedics

BACKGROUND The utility of advanced prehospital interventions for severe blunt traumatic brain injury (BTI) remains controversial. Of all trauma patient subgroups it has been anticipated that this patient group would most benefit from advanced prehospital interventions as hypoxia and hypotension have been demonstrated to be associated with poor outcomes and these factors may be amenable to preho...
